The day of tournament check-in, please have available: a copy of your official roster for the tournament to keep all player cards (signed and with a picture) a notarized medical release for every player a team release form a guest players form if your team is from outside of Indiana, a permission to travel form from your home state Please don't hesitate to contact a tournament director if you have questions about anything!
